Abstract Objective To observe the therapeutic effects of modified Buyang Huanwu decoction onA bstracthemodialysis syndrome and iron metabolism in patients with acute kidney injury ( AKl). Methods 112 AKlpatients were divided into two groups the conventional group and traditional Chinese medicine group usinga random number table method, with 56 cases in each group. The conventional group were treated withsucrose iron assisted maintenance hemodialysis ( MHl) ). The traditional Chinese medicine group weretreated with modified Buyang Huanwu decoction as an adjunct to MHD. The changes in iron metabolism.blood lipids , hemodynamics, and the occurrence of restless leg syndrome ( RlS) were compared betweentwo groups of patients after treatment, Results After treatment, the serum iron ( Sl), serum ferritin ( SF ),and serum transferrin saturation ( 'TFS ) of the traditional Chinese medicine group were higher than those olthe conventional group. Total cholesterol ( TC ), triglycerides ( TG ), low-density lipoprotein cholesterol(LDL-C ), plasma viscosity, whole blood high shear viscosity, and whole blood low shear viscosity wereall lower than those of conventional methods . The incidence of non-twisted leg syndrome in the traditionalChinese medicine group was lower than that in the conventional group at the first, third, and sixth monthof follow-up(P<0.05 ).Conclusion Modified Buyang Huanwu decoction can effectively alleviatehemodialysis induced iron metabolism abnormalities in AKl patients with MHD, promote hemodynamierecovery, and reduce the risk of RlS
